Site Manager Survey, 2025: The site manager survey gathers valuable perspectives on how afterschool programs are supporting youth. The results will help identify successful approaches, address challenges, and guide improvements that strengthen services for young people, staff, and the community.

Youth Survey, 2025: This survey is being distributed to youth who have participated in a summer before or after-school program this year to help us learn more about their experiences in the program, and how this experience may have shaped their educational outcomes as a result. Questions are also asked to demonstrate how supported and cared for the youth feels by the adults who are leading the before/after-school programming.

Family Survey, 2025: This survey is being distributed to parents who have children who have participated in the school year before or after-school program this year to help us learn more about their child’s experiences in the program, and how this experience may have shaped their educational outcomes as a result. We’re also interested in parents’ satisfaction with the program, as well as whether or not they believe their child enjoys attending the program.

Teacher Survey, 2025: This survey is being distributed to adults who know youth at school who have participated in the school year before or after-school program this year to help us learn more how this experience may have shaped their educational outcomes as a result. School adults (i.e., teacher, mentor, counselor, etc.) are asked to evaluate each student's academic behaviors and social-emotional development.

32n OST Site End-of-Year Survey (September 2024): This survey serves as your end-of-year reporting to the Michigan Department of Lifelong Education, Advancement, and Potential (MiLEAP) and the Michigan Afterschool Partnership (MASP).
